Health Information Technologies and Processes

My facility, a critical access hospital is wanting to implement a single page sign-off for receipt of multiple documents, i.e. Consent for Treatment, Release of Information, Notice of Privacy Practices, Patient Rights and Responsibilities, Notice of Visitation Rights, Physician Availability Disclosure, etc.    We would also include the statement "I received or was offered the following documents at the time of my admission to the facility and had them explained to me in a language I could understand".

Does anyone see any problems with going to the single page sign-off rather than the patient having to sign each document individually?

This is a wonderful idea, and I am also interested in seeing what others recommend as well.  We are also a small CAH and are always looking for ways to better the patient experience.

One approach I often see is that each of these areas you described are all on one page, but each has its own line for the patient or patient's representative to initial.

It's a bit of a hybrid approach in putting things on one page...still requires some type of acknowledgement for each of these on by the patient...and easier to use when questions come up if the patient was provided information or was made aware of the items listed.
