Health Information Technologies and Processes

Hello - I know there is a regulation that outlines what is defined on each page of the patient chart.  The patient label perse should have information for each page in the chart.  Can someone tell me what that regulation is so I can find it to quote to a site that I am working with on this topic?  I believe the information is the patient name, account number, hospital/facility name, date of birth, and physician.  And, all this should be defined on each page.  Any assistance would be greatly appreciated.

Very interesting...if you should find any such regulations that are as specific as you described...please do share.  I would be very surprised to see a regulation that was that specific.  There may be "best practices", "industry standards", etc, etc...that may suggest that the patient identifiers you listed should be on each page...but as for a regulation...I would be VERY surprised.

Thank you for your input.  I am sure I have seen this in the past, but not quite sure where I have seen it, but it does make sense that it may have changed since I cannot find any info on it now.  If I find something more then I will be sure to add it to this post for any that review in the future.

I'm no sure about each page of a chart but have you thought to look over the UHDDS Data Set? This is a FDR document that informs you what is to be included.
Attached is copy, don't mind the name of the document UHDDS covers several types of settings.

Hope this shed some light.
