    I am looking at a discharge summary for a patient. The attribution key shows me the following physician entries:

    JD1 - Doe, John E, MD on 7/14/2017 at 2:22 PM

    JD2 - Doe, John E, MD on 7/20/2017 at 12:44 PM

    JD3 - Doe, John E, MD on 7/14/2017 at 2:23 PM

    The physician signature on the note looks like this:

    Doe, John E, MD, 7/1[JD3] 3 [JD2] /2017 [JD3] 4 [JD2] : [JD3] 05 [JD2] PM [JD3]


    I'm concerned because it looks to me like the physician is altering the date/time stamp of his discharge summary. Can an electronic signature  in a discharge summary in Epic be edited in this way?  Should a physician be editing his signature in this way?

    why are the bracketed initials in the attribution key out of chronological order i.e. why does JD2  list before JD3 when the date and time occur after JD3

    Any help would be greatly appreciated.
