    Does anyone have a consent for telehealth/audio/visual for patients you are willing to share?

    We are obtaining verbal consent and the following language is a smart text  inserted in the electronic documentation

    The patient was made aware that this visit is being performed using virtual visit technology. The benefits include efficient medical evaluation and management by allowing the patient to stay in their home or remote site.  The risks/limitations of a virtual visit include but are not limited to the lack of physical exam and possible technology failure. The patient verbalized understanding and provided consent to continue with the visit.

    We are also looking to do a verbal consent with similar language about the risks and agreeing to not record the session as well as ability to revoke the consent and not participate at any time.

    Thanks everyone.  Here is our draft in case it's helpful to someone else. 


    I understand that my ongoing services may be provided using telemedicine. 

    My provider has explained that this may include the use of audio and/or video conferencing technology delivered via telephone, smartphone, tablet and/or computer.


    I understand the potential risks of this technology including interruptions, technical difficulties, and unauthorized access.  To minimize these risks, I agree not to record the session and not share any access links/passwords sent to me by my providers. I acknowledge that I am responsible for protecting access to my devices and accounts. 


    I understand that I may receive a telemedicine meeting link via my email address and that email is not to be used for ongoing communication or urgent requests.


    I understand that services I received using telemedicine will be billed to my insurance company in the same manner as in-person services, where allowed.


    I have had an opportunity to ask questions about telemedicine services and I choose to proceed with receiving services using the telemedicine options/technology described above.

    Our process is very similar to Natalie. We have developed Smart Phrases for our staff to use in our EHR. 



     Patient has verbally consented and authorized Community Health Network to provide and perform care under the direction of me, as the provider, as considered advisable for the patients' health and wellbeing.  This encounter was conducted via a video visit.
