    I am gathering data on chart corrections.  How many cases per month do you have where the wrong patient is registered.  Specifically DOB, name, and/or social security number have been changed to the incorrect patient causing the charts to be "separated".    Thank you.

    If you're looking for benchmarking data; you'll want to narrow this query down to similar organizations.  What's the size and growth rate of your EMPI?  What's the mix of ambulatory, inpatient, ED, urgent care, SNF and ambulatory surgical center patients?  Proportion of pediatric patients?  Immigration status, citizenship and ethnic diversity of your patient population? These factors will all affect your rate of patient identity overlays.

    Something else to keep in mind, with calculating your overlay creation rate, is that it is a factor of patient lookup events and not just patient demographic change, registration or new patient creation events.  Every time anyone in your organization attempts to find a patient in your EHRS; there is the potential for a patient identity overlay. These often compound over time and aren't discovered until someone eventually "corrects" one of the key identifiers like name, birthdate or SSN.  In my experience, overlays can be ongoing for months or years before discovery.  This makes keeping accurate real-time statistics challenging as well.
