    Hello colleagues,
    I'm reaching out to ask how are you using the 'Poor Quality Stamp' to identify documents you receive which are not the best quality? Currently, we have our HIM Operations Scanning Technicians stamping each page in a batch of poor quality documents.  Please share how your your organization is processing these documents/batches of poor quality documents:
    • Are you stamping only the 1st page of a set of documents?
    • Every page poor quality page?
    •  Same patient
    • Document Description

    We are currently looking at all of our scanning processes and working to streamline them. During a team meeting, the discussion of the 'Poor Quality' stamp or the vendor's 'Copied from Poor Quality Original' stamp came up. What are other organizations doing?

    We currently stamp every page (even external) if it is illegible with the "poor quality" stamp on the same patient.  I am interested in how others are doing it.

    We no longer have a stamp because our software vendor has an internal "button" that will indicate "Best copy obtainable" at the top of the document. This appears at the top of each page of the document. Prior to this software, we would stamp each page.

    We currently stamp each page that is not the best quality with a stamp "poor original".  We do this at the time of chart prep.  If the QC person feels any were missed, they use the electronic stamp with the same name in the electronic record.  We stamp them before scanning because we do not do 100% QC.
