    The practice of Interdisciplinary Team Notes: There is an monthly and annual interdisciplinary team notes that are documented by various disciplines/people on one note. This note does not get signed but the user who submitted the entry would be posted with the user name, date and type of note.What are your thoughts on this practice?

    Hi Lori,
    thanks for replying to my post. Multiple users document on this one note (not report). An initial monthly note is created by a designated person in a discipline and another designated person from another discipline would then add or "amend" the original note. The note captures the user who is creating or adding to the note but they are not able to sign since the note does not belong to one person and/or discipline.
