    Hello -  I'm looking for guidance on coding pulmonary insufficiency when it is in a patient who also has aortic, mitral or tricuspid insufficiency.  I know that pulmonary insufficiency defaults to nonrheumatic unless it is specified as rheumatic.  The index for aortic, mitral and tricuspid insufficiency has subheadings of "with" that lead you to the combination I08... codes, but for pulmonary insufficiency, it doesn't have a subheading "with."  Am I correctly interpreting this to mean that if a patient has both pulmonary insufficiency with either aortic, mitral or tricuspid insufficiency (or a combination of those three), I would be correct in using I37.1 for the pulmonary insufficiency in addition to the correct code for the others?  If that is the case, in what instances would I08.8 (other rheumatic multiple valve diseases) be used?  An example:  I am looking at an echocardiogram that documents aortic, mitral, and tricuspid insufficiency, in addition to pulmonary insufficiency.  Would this be correctly coded as I08.3 and I37.1?
