    Hi Sherika,

    I believe the Guideline ODX#2 refers to an older guideline. It is also an ISO standard for data.

    I noticed you are a CDIP. If you are a member of ACDIS, they can definitely help you with finding that particular guideline. In the meanwhile, if you can access this:

    It is a discussion from 2014 containing your reference. You can also search and find an AHA from 2002.

    Hope this points you in the correct direction to find this particular guideline.

    Thank you so much Kelly. I was just trying to pull it since the coding clinic referenced it. Between the guideline and coding clinic I was able to demonstrate that a confirmed diagnosis does not have to be documented in the discharge summary to code it. It should be documented consistently and not conflict with the attending's documentation to be coded. Thank you for your help in this matter!

    I'm glad you asked.  ODX #2 comes from an ancient version of ICD-9-CM coding guidelines.  At one time, long ago, the guidelines for reporting other diagnoses (ODX) were categorized as​​ ODX #1, ODX #2, ODX #3, etc.  You can review these ancient, outdated guidelines in Coding Clinic for ICD-9-CM Second Quarter 1990 but don't use these outdated guidelines for coding in ICD-10-CM.  Use instead the ICD-10-CM Official Guidelines for Coding and Reporting.  ODX #2 was last mentioned in a Coding Clinic article in Second Quarter 2000, but unfortunately many people cite this Coding Clinic article without paying attention to the fact that ODX #2 comes from an outdated set of ICD-9-CM coding guidelines.​​​​
