Leprosy vs Sequela

    How would you code for a patient who is actively receiving treatment for Leprosy but comes in with a Type 2 Lepra Reaction? My Providers are coding it as B92- Sequelae of leprosy. However when I look under that code it says to "code chronic current infections to active infectious disease as appropriate." So would you not code the Leprosy A30.9?

    I haven't coded Leprosy in a very long time, but based on web searches to better understand a Lepra Reaction, this occurs during treatment, as it seems to be a immune or hypersensitivity reaction.  I would code the Leprosy.
    Not sure I would code Lepra Reaction as a sequela as for coding, this implies the patient no longer has the Leprosy infection.  Perhaps code the Lepra Reaction to T78.4- category.  You may want to query your physician to find out if the treatment is prophylactic or for the ongoing infection, as Leprosy takes a long time to treat, unless the record indicates otherwise.
