I have a couple scenarios regarding the coding of palliative radiation.

First scenario:  Patient is transferred to our facility with leg weakness due to compression of spinal cord secondary to widely metastatic prostate cancer to bone. Patient is receiving palliative radiation prior to discharge to hospice care.

Second scenario:  Patient presented to the ER for acute onset of back pain.  Finding on the MRI patient has acute spinal cord compression due to metastatic bone widely metastatic with unknown primary site.  Patient deferred to radiation oncology where patient agreed with palliative radiation.

Your direction and feedback would be greatly appreciated.
