    when the patient was readmitted after an initial acute pulmonary embolism for other reason i.e chemotherapy and all the provider states is recent pulmonary embolism.  The patient is still being treated with anticoagulants. No CT-thorax or Doppler study was done during this visit. I have sent physician query and physician answered it as recent PE/DVT treated with LMWH.

    How should we report PE/DVT ?

    Was the chemo for a cancer? You would code the chemotherapy (Z51.11), then the cancer, and then...

    You say "recent PE" so it is no longer current? If so that would be Z86.711 (PMHx PE), appropriate since chemo can cause PEs.

    (Since he is still on anticoagulants Z79.01 may be appropriate, though I don't know if 22 days constitute long term, or if the medicine is to be continued long term).

    Based on the information you provided I would code it as a history of PE/DVT. To me "recent" sounds like it is not considered to be a current diagnosis.

    ​Hello!

    For what it is worth, I agree with the other contributor.  Since you queried and the physician responded "recent", not "current" or other terms to that effect code to history of PE.

    The patient will probably be treated with anticoagulants as a preventive measure for quite some time.
