    We receive a lot of outside records from other providers/facilities.  Once these are filed/scanned into our agency records we have been adding our patient number and unique record number.  This is quite labor intensive.  What do others do?

    Hi, We do not add our facility identifiers to outside records. I work in a special hospital, which is a residential facility, and we get a large volume of paper when our patients go to the local acute care hospitals (now that they are all on Epic). Our physicians do not want to keep all of that, so we have a procedure for what we keep. Most of it is discarded. We have electronic access through the HIE, so we can print and scan the parts we want, such as consults and test results.​

    Thank you.  It is helpful.  We receive a lot we don't need too, but unfortunately we don't have the manpower to sift through sometimes 100s of pages.

    When we receive outside records, we will review the chart to see which provider requested them. That provider must then initial each page he/she wishes to have scanned in as part of the record which holds the physicians accountable for the information on each page they initial. If one of our physician didn't request the records, we do not scan them.
