How to account for Patient disclosure

Hello, I need some input on how other facilities are handing patient disclosures through secure channels. Below is an example

I need to communicate to a workforce member regarding patient x. I IM or Chat to my co worker about patient X disclosing PHI as we communicate.

When the patient requests an accounting of disclosures how do you account for IM or Chats about the patient that took place that is not being captured?

Do you consider this incidental disclosures and therefore not needing to document it as an accounting of disclosure?

I would not consider these disclosures, just inter-office communication about patients.......

I agree with the others, this part of hospital operations and not part of the AOD requirements.
