    To address Electronic Health Record implementation success, participation is requested in a brief survey.

     As a student at Indiana State University, I will be gathering information and analyzing the data received. This project is being conducted under the supervision of Dr. Mehran Shahhosseini at Indiana State University who can be contacted at mehran.shahhosseini@indstate.edu. This survey has been posted to the AHIMA Engage online communities, and is a product or request from a private individual and not one from AHIMA/AHIMA Foundation unless specifically noted as such. Neither AHIMA nor the AHIMA Foundation sponsor or endorse survey information or this request. 

    It is anticipated that this survey will take approximately 20 minutes and participation is voluntary. Participants have the right not to answer any questions and to stop the survey at any point. Identifiable data will not be used in reporting of results, and aggregate data will be used whenever possible. The following measures will be taken to ensure the confidentiality of all data received by ensuring no personal information will be collected, including IP addresses and/or any other contact information. This survey has gone through the Institutional Review Board (IRB) review process and has received EXEMPT FROM IRB REVIEW status from Indiana State University for IRB protocol number 1443817-2. 

    Funding for this survey has not been received, and incentives will not be paid. Follow-up contact with survey participants will not be made. 

    Results will be disseminated via a dissertation publication, and as required all documentation will note the following:

     Information gathered for this study is the result of a survey or other collection tool posted by a visitor to an AHIMA Engage online community.
