I was always lead to believe that Ethics Consultations should
not be part of the patient's health record. However in the past few years, our corporate sponsor has moved us in another direction with the worksheet being part of the health record.

I am surveying the AHIMA community. Do your organizations make Ethic Committee documentation a part of the patient's medical record?
Currently our Ethics Consultation Request Worksheet and Assessment is not part of our electronic health record. Per our policy the Consultation Request Worksheet and the Ethics Committee Assessment form are stored in the Medical Staff office. The worksheet was developed as an internal request and tracking form and would not normally be a part of the patient's medical record. The information on the worksheet should match information available in EHR. The physician, clinical staff and/or social services should document their follow-up in response to the Ethics Committee consultation and/or recommendations.
