    ​Good Afternoon,

    We were recently surveyed by JC and one of their findings were that the HIPAA Authorization form should have a time line next to the date. They cited RC   Our understanding was that the time was not required for this type of form.  Can you confirm if the time is required or optional per your facility policies...has anyone else received a citation/findings of which they are requiring we do a plan of correction.

    I have not worked for a JC accredited facility for years.  I work in a community mental health center.  We do not time our authorizations.  We do date them.  What is the purpose of the time?

    ​The JC cited RC 01.01.01 EP 13, their reasoning was ALL entries in the medical record must be signed dated and timed.  Our forms only require signature and date no time.  I did not agree with the finding and was forced to do a POC...The HIPAA Release Form/Authorization is not an entry in the record but more so of a document that helps release information...  Our state form does not have the time either ...

    Hello,

    My facility is JC accredited and according to the California Hospital Consent Manual, there does need to be a space for the time. This may vary depending on which state your facility is in, but if you look in the Hospital Consent Manual for your state, you should find the information you need. Hope that helps!

    Call me out on this if wrong but I thought authorization actually did not have a regulated date and time limit.  It is access request that actually has a time and date limit.

    We never receive authorizations from other healthcare providers that have the time documented.  In my 30 plus years in the profession I have never seen it required.

    For an authorization to be valid, it must have an expiration date or event.  I have been at facilities that had verbiage in their authorization forms stating that the authorization expired upon initial release of the information unless otherwise stated.

    The Privacy Rule requires that an Authorization contain either an expiration date or an expiration event that relates to the individual or the purpose of the use or disclosure. For example, an Authorization may expire "one year from the date the Authorization is signed," "upon the minor's age of majority," or "upon termination of enrollment in the health plan."
