    I'm looking for some guidance, I took a new role recently at a small facility that has an HIM department but no real policies in place. I have created some along the way but can't truly remember all the policies that should be in place. Any suggestions on where to find some information or where to even start?

    Good Morning Kristin - I would suggest you take a look at this book for references to the policies and procedures you would need to build.Health Information Management: Concepts, Principles, and Practice, Fifth Edition by Pamela K. Oachs, MA, RHIA, CHDA, FAHIMA & Amy L. Watters, EdD, RHIA, FAHIMA. I have the previous version and it has been a great resource. Another great place to search of course is our Body of Knowledge. You should be able to find P&Ps, white papers, and time studies there based on the function you are defining. Best of luck.

    You may also start by reviewing the accreditation standards for which ever organization they use, CMS regulations and state regulations. 


    Are you also considered the Privacy Officer?  That brings in a lot more to consider.

    I'm attaching a checklist for the privacy rule and breach notification rule.  You need policies that address each item.  Several regulations can be combined into one policy.

    You also want to consult the State regulations, in addition to accrediting bodies. Think about what comprises your documentation, and the timeliness standards. Who can document in the medical record? Record retention. Procedures for day-to-day departmental operations. And as mentioned, if you are responsible as the privacy officer, all of the required HIPAA policies need to be in place as well.
