    Quick poll please - there are several categories that are always considered a planned readmission.  As such, are you reporting the PRA for your disposition code or is it not necessary since they are always considered planned regardless?

    ​Hi Lynn,

    The proper reporting of discharge disposition codes is maintained by the National Uniform Billing Committee (NUBC) which is available for purchase.

    However, it is my understanding that it is never appropriate to designate a discharge disposition based on the category of the patient.

    The patient record has to clearly document that the patient is being sent home, but is expected to return for a specified surgery.

    This has to be documented in the discharge plan.  It cannot be applied based on the patient's category or diagnosis.

    Your business office may have a copy of the UB-04 manual for your review.
