    As you may already know, NED stands for "no evidence of disease". If the patient is no longer receiving active treatment for the condition (i.e. chemotherapy, radiation, etc.) you would report this diagnosis using one of the “personal history of malignant neoplasm" codes. Most personal history codes cannot be reported as the first-listed/ principal diagnosis. If the encounter was to assess or "follow-up" on the lymphoma, I would assign Z08 (Follow-up of treatment for malignant neoplasm) for the first-listed/ principal diagnosis. I would then report the personal history of malignant neoplasm, stem cell transplant status, followed by any history of chemotherapy, history of radiation therapy, chronic conditions, etc. I hope this helps.
