Troponin Icd 10 CM

    Hi i was wondering what is the correct DX for Elevated troponin?
    Based of what I've read troponin is a plasma protein.  But when i did a research people choose one of those ICD 10 CM code.
    which would be the correct code?

    Elevated troponin really isn't a diagnosis – it's a laboratory result.  You need a physician interpretation of what it indicates.  I know there are codes for it but you may find issues with reimbursement.

    Our compliance team has advised us to use R79.89 when we have this as one of our diagnosis.

    You are correct.  Troponin is defined as a plasma protein.  Per the AAPC website and other sources, the correct code is R77.8.  I assume you are coding from a physician's diagnostic statement and not directly from labs.  I am not sure if you are coding for an inpatient record versus some other setting.  For inpatient, if the possible underlying reason for the elevated troponin is documented (usually an AMI) assign that code and do not code R77.8.  For other settings, assign R77.8.

    According to a response we got back from AHA, elevated troponins would be coded as R74.8, Abnormal levels of other serum enzymes.

    Hello Nancy,

    Could you please share a bit more in terms of documentation from the AHA on the use of R74.8 for elevated troponin? I have been 'dinged' in an audit for having used this code, and need some sort of definitive proof. Thank you so much!
