Coding, Classification & Reimbursement

1.  Coding stent placement/inpatient

Posted 7 days ago
Need assistance with IP coding of cardiac stent placement.  One stent placed in the L Circumflex and the
other placed in the OM 1.  Since there were rule changes in 2017 from coding number of arteries to number
of sites, are these 2 arteries considered one (the OM 1 branches off from the LCX) or are they considered
two separate arteries?  Thank you.

Coding Clinic 4Q 2016 p 82 lists coronary arteries and their branches that are counted as individual arteries for coding purposes.  This list includes circumflex and obtuse marginal as separate arteries.

  • Right coronary artery (RCA)
  • Right marginal
  • Right posterior descending
  • Left main coronary artery (LMCA)
  • Left anterior descending branch (LAD)
  • Diagonal
  • Septal
  • Left circumflex (LCX)
  • Obtuse marginal (OM)
  • Posterior descending
  • Posterolateral

Dear Yvonne,

In order to be sure I would look at the diagram in the CPT manual which illustrates this very clearly.

Good Luck

Thank you, Jodi.  Since I am an IP coder, I do not have the current CPT manual.   There is an entry in CC that
lists each coronary artery that can be coded separately.
