    Good Afternoon, All -

    After patient records have satisfied the necessary retention laws, what type of policies do other's have for documentation of destruction?
    I am looking for a more efficient process than our current one, which is logging onto a spreadsheet.
    Does anyone else simply document in their practice management systems the date of destruction?
    (This is for ASC's)

    We have a certificate of destruction and we use it by batches - not individual records.   Based on our policy and review of the patient visit history, we can substantiate easily if something has been destroyed.

    ​Hi,
    Somewhat related but a little different.  How are others handling retention of electronic records?  Does anyone have a policy/procedure they would be willing to share outlining retention practices of electronic records?
