    I currently work for an organization who has multiple satellite offices and unfortunately one will soon be closing.  We are tasked with having to fulfill out patient request, move all paper PHI to an offsite location and do a final HIPAA compliant walkthrough.  Historically, we used a handwritten checklist when relocating, however for this situation we would like to use something a bit more encompassing.
    Does anyone know of a HIPAA compliance checklist for closing a practice?  Or perhaps and resources out there that could guide us in the right direction?

    Hi Brittany,

    I've never seen a HIPAA-shutdown checklist customized just for that.  For something so specific, a 3rd party specialist (outside consultant) might be hired, or you could try to assemble your own and create a hybrid from various "HIPAA checklists" and facility-shutdown checklists that are widely available on various websites.

    I'd recommend contacting someone or an entity who specializes in storage, destruction, and removal of PHI for detailed questions.  It'd be very demanding to create a truly valid and substantive but generic HIPAA shutdown checklist that could would be applicable across a broad array of setting, facility, and provider types.
