    Can anyone share any regulations, policies, or other pertinent information regarding physician documentation for a swing bed patient?   We had a patient discharged with only the H&P in the chart for an 8 day stay.   My previous supervisor told me that a physician should see and document on a swing bed patient every 3 days, but I cannot find any regs to support that – not even our own in-house policy.

    ​Look at Medicare 482.58, special requirements for hospitals providing Swingbed Services.  They outline what is required.  Basically an admission H&P can be documentation from one hospital to another for Swingbed services, but there must be documentation of admission orders.  They don't set guidelines for progress notes, just as dictated by the patient's condition, however a physician cannot charge for services provided without one.  Our internal policy states progress notes must be every 3 days.  There needs to be a discharge summary though that contains a recapitulation of the resident's stay, final summary of the patient's status, post-discharge plan of care, very similar to acute care patients.
