1.  Should Registration scan Consent to Treatment Forms?

Hi All,
I am trying to see what the community's best practice is for scanning Consent to Treatments?  We are asking that Registration scan it because it is at what is considered "point of care". Currently, we do no receive these consent forms in the HIM dept until the patient is discharged from OP or IP service and it could be days later, after we have already scanned the patient's visit paperwork in the EMR.  This increases risk of losing the document, the document is not with the original visit paperwork and sometimes it is not received at all.  Are other scanning this at registration or waiting for the document to come down later?  Keeping in mind the patient's chart is already open at registration and the scan will take a few seconds to execute vs the document coming later, having to pull up the patient's chart, modify the documents and then scan it into the record.
Please share or advise who has this responsibility at your facility.

Our admitting department (registration) has been scanning the consent to treat as soon as it is signed.  This signed document then appears in the EMR.  HIM does not receive any paperwork since it is already scanned.

Our general consent and promissory note is signed and scanned at registration. The specific consents that may be done during the course of a patient's stay get scanned in HIM after discharge.

We do the same.  We scan the consent for treatment in registration and then we run a report the following day to ensure all of the consents were scanned into the system.  We then receive the consents in Medical Records at discharge and we'll do a quick check to make sure they are there.

Our registration department scans the consents, advance directory acknowledgment, and any IDs, orders, or other documents at the time they are signed.  Consents are sent to HIM with the record and are re-scanned if necessary (scanned incorrectly, missing labels, illegible, etc.).
